Welcome to the Terms and Conditions of SALT Physiotherapy and Rehabilitation.

At SALT Physiotherapy, we offer mobile physiotherapy services tailored to meet the needs of seniors and older adults on the Sunshine Coast. We strive to provide exceptional home-based physiotherapy, aged care physiotherapy using home care packages or self funded, and NDIS-funded services. Please take a moment to review our terms and conditions before booking a session. These terms are intended to outline our service offerings, including our cancellation policy, travel fees, and other important service details.

1. Introduction

By accessing or using our services, you agree to be bound by these Terms and Conditions, which form a legally binding agreement between you (the client) and [Your Business Name] (hereinafter referred to as "we," "our," or "us"). These Terms and Conditions govern your use of our physiotherapy services.

2. Our Services

We provide mobile physiotherapy services for seniors and older adults on the Sunshine Coast. Our services include, but are not limited to, physiotherapy consultations, assessments, treatments, rehabilitation, falls prevention, mobility training, and exercise programs. We provide services in clients’ homes, recreation facilities such as swimming pools or aged care facilities.

3. Client Responsibilities

  • Medical Information: You agree to provide accurate and up-to-date information regarding your medical history, current health conditions, and any medications you are taking.
  • Cooperation: You agree to follow the physiotherapy treatment plan provided and to inform your physiotherapist if you are unable to do so.
  • Cancellations: You agree to notify us of cancellations or rescheduling at least 24 hours in advance. Late cancellations or no-shows may incur a fee (as outlined in the cancellation policy).

4. Payment and Fees

  • Fees: The fees for our services will be discussed and agreed upon during your initial consultation. Payment can be made via bank transfer only. We also accept NDIS and My Aged Care Package funding (if applicable).
  • Cancellation Fee: If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice. Cancellations made with less than 24 hours’ notice will incur the full cost of the scheduled session, including the travel fee from Mount Coolum.
  • Travel Fee: A travel fee will apply for all appointments, starting from Mount Coolum. The fee will be calculated based on the distance to the appointment location and will be agreed upon during the booking process.
  • Aged Care PackagesIf you are accessing services through an Aged Care Package, we will work with you or your representative to tailor a package that suits your needs. Any services provided under this package will be billed according to the agreed terms, and we will ensure that we are compliant with the My Aged Care guidelines.

5. Cancellations and No-Show Policy

  • If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice. Cancellations with less than 24 hours’ notice will incur a cancellation fee of the full amount.
  • No-shows will be charged the full session fee inclusive of travel.
  • If we need to cancel or reschedule an appointment, we will do our best to provide as much notice as possible and offer alternative scheduling options.

6. Privacy and Confidentiality

We take your privacy seriously. We collect personal and health-related information for the sole purpose of providing physiotherapy services. All client information is kept confidential and secure, in accordance with the Privacy Act 1988 (Cth). We will not share your information with third parties without your consent unless required by law.

For clients in aged care, we will also comply with any relevant Aged Care quality and safety standards.

7. Liability and Consent

  • Client Consent: By booking an appointment, you consent to receiving physiotherapy treatment. You acknowledge that physiotherapy treatment can involve some risks, and you agree to inform your physiotherapist of any concerns or changes in your health.
  • Limitation of Liability: While we strive to provide the highest standard of care, we are not liable for any injury, loss, or damage that occurs outside the scope of the agreed physiotherapy treatment plan.

8. Health and Safety

We are committed to providing safe and professional care. Clients are expected to inform us of any pre-existing conditions, allergies, or health concerns that may affect treatment. It is your responsibility to follow the advice provided and seek medical clearance if needed before beginning physiotherapy.

9. NDIS Clients (if applicable)

For clients receiving NDIS funding, we are registered providers and will ensure all services are in compliance with NDIS guidelines. Payment will be processed according to the terms outlined in your NDIS plan.

10. Website Use

By visiting or using our website, you agree to use the content for personal purposes only and not for commercial gain. You are prohibited from copying, distributing, or using any content without express permission. We make every effort to ensure our website content is accurate, but we do not guarantee the accuracy of information.

11. Dispute Resolution

If any dispute arises, we encourage you to contact us directly to discuss the matter and attempt to reach a resolution. If an agreement cannot be reached, both parties may seek resolution through an appropriate mediation process.
